Identify
A qualification gap, defect or approaching deadline is recorded against the person or unit it belongs to.

Act
An accountable person and a due date are assigned. The item stays open until it is resolved.

Verify
Completion is recorded with evidence — a repair order, receipt, report or signed record — for the next review.

Measure the work. Keep the evidence.
The pilot records comparable periods for the same fleet before and after the process is in use. These are operating measures, not a safety rating, and not legal, insurance or accounting advice.
Principles
- Compare the same cohort with the same definitions in the baseline and the follow-up period.
- Keep the source records behind every number: inspection reports, maintenance logs, load records.
- Observations are append-only. Corrections are recorded as new observations, not edits.
- Missing records remain missing. Do not reconstruct a positive result after the fact.
- Report limitations next to results, including fleet size and the length of each period.
Measures
- Defects reported per 10,000 miles
- Defects found in inspections or reported by drivers, divided by total miles.
- Corrective actions closed by due date
- Share of actions opened in the period that were completed with evidence by their due date.
- Overdue items at period end
- Open actions and expired documents on the last day of the period.
- Miles per power unit
- Total recorded miles divided by the number of power units, for context.
